Self Report a Traffic Crash

Drivers of a vehicle involved in a crash resulting in damage to a vehicle or other propertywhich does not require a law enforcement report are eligible to self-report the crash in lieu of a law enforcement officer completing the crash report.

Ensure all relevant sections are completed accurately. Additional guidance can be found on page two of the form.

Step 4.Send the Completed Form

Once filled out, email a copy of your report to SelfReportCrashes@flhsmv.gov.
